Provisions |

The school carries Accidental Medical Insurance which covers any excess over your personal insurance. The school is also covered for liability. |

Refund requests can be submitted to the Day Care Board for approval. |
| |
Modifications to the original agreement shall be made whenever circumstances covered in the agreement change, and shall be dated and signed by the persons specified. |

The licensee shall retain the original copy of the agreement and shall provide copies to the parent and/or to the authorized representative, if any. |
| |
The department or licensing agency shall have the authority to interview children or staff, and to inspect and audit child or facility records without prior consent. |
| |
The licensee shall make provisions for private interviews with any child(ren), or and staff member, and for the examination of all records relating to the operation of the facility. |
| |
The
department or licensing agency shall have the authority
to observe the physical condition of the child(ren), including
conditions which could indicate abuse, neglect, or inappropriate
placement, and to have a licensed professional physically
examine the child(ren). |
